Earning Reward Points
How do I earn reward points?

You can earn Centennial Rewards Points by making certain healthy choices. Centennial Care Rewards may be subject to certain exclusions and restrictions based on the Healthcare Effectiveness Data and Information Set (HEDIS), as modified by your health plan. HEDIS® is a registered trademark of the National Committee for Quality Assurance (NCQA). Call us at 1-877-806-8964 (TTY 1-844-488-9722) for more information or if you have questions.

The specific ways you can earn points are listed below.

How do I earn Healthy Smiles Rewards?

Centennial Care members may be able to earn rewards for yearly dental visits. The following rules apply:

Age 18 and older: Enrolled adults may qualify for $25 (250 points) in rewards each calendar year for going to at least one dental visit.

Age 2-17: Your enrolled children may qualify for $35 (350 points) in rewards each calendar year if they have at least one dental visit.

Centennial Care Rewards may be subject to certain exclusions and restrictions based on HEDIS standards, as modified by your health plan.

How do I earn Step-Up Challenge Rewards?

Centennial Care members may be able to earn up to $50 (500 points) in rewards for completing the Step-Up Challenge walking program

Age 10-17: Rewards will be made to the parent or legal guardian.

Age 18+: You can earn the Step-Up Challenge rewards in your own account.

Register for the 3-Week Step-Up Challenge: Login to your Member Rewards Portal and register online. You can also register by phone at 1-877-806-8964 (TTY 1-844-488-9722). We will mail you a free Step-Up Kit with a pedometer to track your steps.

3-Week Challenge: Earn $25 (250 points) in rewards if you successfully track the minimum number of daily steps for 3 consecutive weeks.

Week 1 Goal: Average 5,000 steps a day.

Week 2 Goal: Average 7,500 steps a day.

Week 3 Goal: Average 10,000 steps a day.

Register for the 9-week challenge if you successfully complete the 3-week challenge.

9-Week Challenge: Earn an additional $25 (250 points) in rewards if you successfully track the minimum number of steps for 9 consecutive weeks.

Set your own goal of 5,000 or more daily steps to track each day.

Week 1-9: Track your daily goal for at least 6 of 7 days each week.

Centennial Care Rewards may be subject to certain exclusions and restrictions based on HEDIS standards, as modified by your health plan.

How do I earn Healthy Pregnancy Rewards?

You may be able to earn $100 (1,000 points) in rewards if you are pregnant and join your health plan's prenatal program.

Age 18 and older: You can earn the prenatal reward in your own account.

Age 17 and younger: The prenatal reward can only be given to your parent or legal guardian.

Centennial Care Rewards may be subject to certain exclusions and restrictions based on HEDIS standards, as modified by your health plan.

How do I earn Asthma Management Rewards?

You may qualify for up to $60 (600 points) per calendar year in rewards for refilling your child's asthma controller as prescribed. The following rules apply:

Age 5-17.

Diagnosed as having persistent asthma.

Earn $5 each time you refill your child's asthma controller as prescribed, up to $60 (600 points) per calendar year.

Most points are awarded within 3 months after you complete a healthy behavior. Points are awarded based on the calendar year in which they are earned (e.g., the refill prescription date). Centennial Care Rewards may be subject to certain exclusions and restrictions based on HEDIS standards, as modified by your health plan.

How do I earn Diabetes Management Rewards?

Centennial Care members with diabetes may be able to earn up to $60 (600 points) in rewards each calendar year.

Age 10-75: You may be able to earn rewards for taking any of the three tests as directed by your doctor if you have a diagnosis of type 1 or type 2 diabetes:

HbA1c test: $20 = 200 points per calendar year.

Eye exam: $20 = 200 points per calendar year.

Nephropathy exam to check your kidney function: $20 = 200 points per calendar year.

How do I earn Bipolar Disorder Management Rewards?

You may qualify for up to $60 in rewards each calendar year if you refill your bipolar disorder medication as prescribed. The following rules apply:

Age 18-64.

Diagnosis of bipolar disorder.

Earn $5 each time you refill your bipolar disorder medication as prescribed, up to $60 (600 points) per calendar year.

Most points are awarded within 3 months after you complete a healthy behavior. Points are awarded based on the calendar year in which they are earned (e.g., the refill prescription date). Centennial Care Rewards may be subject to certain exclusions and restrictions based on HEDIS standards, as modified by your health plan.

How do I earn Schizophrenia Management Rewards?

You may qualify for up to $60 in rewards each calendar year if you refill your schizophrenia medication as prescribed. The following rules apply:

Age 18-64.

Diagnosis of schizophrenia.

Earn $5 each time you refill your schizophrenia medication as prescribed, up to $60 (600 points) per calendar year.

Most points are awarded within 3 months after you complete a healthy behavior. Points are awarded based on the calendar year in which they are earned (e.g., the refill prescription date). Centennial Care Rewards may be subject to certain exclusions and restrictions based on HEDIS standards, as modified by your health plan.

How do I earn a Bone Density Testing Reward (Women age 65 or older)?

You may qualify for a one-time reward of $35 (350 points) for getting your bone density checked for osteoporosis. The following rules apply:

Women age 65 or older.

Bone mineral density (BMD) test during the current year.

Centennial Care Rewards may be subject to certain exclusions and restrictions based on HEDIS standards, as modified by your health plan.

My Children's Points
Do my children earn points in their own account?

No. As their parent or legal guardian, you earn points in your account for all Centennial Rewards activities that your children complete, such as going to the dentist for their annual visit or refilling their asthma controller as prescribed.

What happens with my children turn 18?

Your children can register their own Centennial Rewards account when they turn 18. Any new points earned by your children will then be added to their account. Any unused points earned before they turned 18 will remain in your account. New accounts can be registered online or by calling 1-877-806-8964 (TTY 1-844-488-9722).

Awarding Points
When are points awarded?

Most reward points are awarded within 3 months after you have completed a Centennial Rewards activity.

When will I see my points?

You will generally be able to see your points within 3 months after you have completed a Centennial Rewards activity. Go to the >Track page to view your points.

Spending Points
What can I buy with my Centennial Rewards Points?

You can use your Centennial Rewards Points to buy healthy items from the Centennial Rewards Catalog.

When do my reward points expire?

The points you earn in one year must be spent by the end of the following calendar year. For example, points earned in 2016 will expire on December 31, 2017.

How much are my reward points worth?

There is no dollar value for your reward points. The points are awarded when you complete specific Centennial Rewards activities. Each point you earn counts as 10 cents in rewards. For example, you can earn 250 points when you get an annual dental visit. That is worth $25 in rewards (250 x $0.10 = $25).

When can I spend my reward points?

You can spend your points as soon as they are awarded. Most reward points are awarded within 3 months after you have completed a Centennial Rewards activity.

What can I buy with my reward points?

You can use your reward points to buy items from the Centennial Rewards Catalog, such as a fitness ball, water bottle, yoga mat, and more!

Do my reward points expire?

Yes. The points you earn in one year must be spent by the end of the following calendar year. For example, points earned in 2016 will expire on December 31, 2017.

What items are available in the catalog?

The Centennial Rewards Catalog includes fitness balls, yoga mats, water bottles, first aid kits, and other cool stuff.

Will items in the catalog always be available?

No. Catalog items may be out of stock. If an item is out of stock, we will ask you to select another item or send you a similar item.

Will items in the catalog always be the same?

No. Item colors, designs, and sizes may vary from what is shown in the catalog.

How will I know when I have earned points?

Go to the Track page to see any points you have earned. You will generally be able to see your points within 3 months after you have completed a Centennial Rewards activity. [Link to Track]

Where can I get a Centennial Rewards Catalog?

You can view the catalog on the Shop page of this website. You can also have a printed catalog mailed to you by calling us at 1-877-806-8964 (TTY 1-844-488-9722).

How can I buy items from the Centennial Rewards Catalog?

You can buy items from the catalog online or by phone. You can view the catalog items online or call us at 1-877-806-8964 (TTY 1-844-488-9722) to order by phone. Our Member Services Agents are available from 8 a.m. to 5 p.m. Mountain Time, Monday through Friday.

How fast will my order be shipped?

Most items are shipped within 10 days after you place an order. Please allow up to 10 additional days for your order to be delivered.

Communication Preference
What is a communication preference?

You will be sent a message each time you earn points. Member Services may also contact you when you place an order or for other reasons. You can choose your communication preference when you register for Centennial Rewards: email, web, or by mail.

How do I change my communication preference?

You can change your communication preference on the My Account section of this site.

Health Center
Where can I get support to help improve my health?

Login and then go to the Health Center to find tools, calculators, and resources to help you learn about and improve your health. Your health plan also provides many resources on their Member Portal.

What if I'm ready to get an annual dental visit?

You can earn $25 (250 points) for your annual dental visit. Plus, for each enrolled child you can get an extra $35 (350 points) for their annual dental visit. Learn more about the importance of dental health.

What if I want to join the Step-Up Challenge

You can earn $25 (250 points) in rewards for successfully completing the 3-Week Step-Up Challenge walking program. You can earn another $25 (250 points) for successfully completing the 9-Week Step-Up Challenge. You must start with the 3-week challenge. You can register online or by phone at 1-877-806-8964 (TTY 1-844-488-9722). We will mail you a free Step-Up Kit with a pedometer to track your steps.

What if I am pregnant?

We will provide you with resources, tools, and support during your pregnancy. You can also earn $100 (1,000 points) for enrolling in your health plan's prenatal program.

What if I have diabetes?

We will provide you with resources, tools, and support to help you manage your diabetes. You can also earn $60 (600 points) for managing your diabetes.

What if my child has asthma?

We will provide you with resources, tools, and support to help you manage your child's asthma. You can also earn $60 (600 points) for refilling your child's asthma medication.

What if I have bipolar disorder?

We will provide you with resources, tools, and support to help you manage your bipolar disorder. You can also earn $60 (600 points) for refilling your bipolar disorder medication.

What if I have schizophrenia?

We will provide you with resources, tools, and support to help you manage your schizophrenia. You can also earn $60 (600 points) for refilling your schizophrenia medication.

What if I'm a woman aged 65 or older?

You can earn a one-time reward of $35 (350 points) by getting your bone density checked to test for osteoporosis.

Ordering Reward Items
What if the item I ordered is damaged?

Damaged items must be returned within 10 business days after the date of the postmark. Call us at 1-877-806-8964 (TTY 1-844-488-9722) to request a return label or ask the carrier to return the package to sender. The item must be returned in the original packaging. We will replace the damaged item with the same item if it is available. You may need to choose a different item if the item you ordered is no longer available.

What if I don't like the item I ordered?

All orders are final. No returns or exchanges are allowed if you decide you don't like the item you ordered.

What if I don't receive my order?

You should make sure that you provide the correct address when you place an order. Items will not be re-sent if our tracking report shows that the package was delivered to the address you provided. Call us at 1-877-806-8964 (TTY 1-844-488-9722) if you provided the correct address and did not receive your order.

What if I used the wrong address for my order?

Check your address carefully when placing an order. Items will not be re-sent if you gave us an incorrect address.

Protecting Your Information
Is there a cost for joining Centennial Rewards?

No. There is no cost for eligible members to join the Centennial Rewards Program. Beware of potential scams. Do not provide your bank account, credit card, or debit card numbers to anyone claiming to represent Centennial Rewards. Please report any suspicious calls or letters to us at 1-877-806-8964 (TTY 1-844-488-9722).

Is there a cost for using my Centennial Rewards Points?

No. There is no cost for spending your Centennial Rewards Points. Do not provide your bank account, credit card, or debit card numbers to anyone claiming to represent Centennial Rewards. Please report any suspicious calls or letters to us at 1-877-806-8964 (TTY 1-844-488-9722).

What if I get a phone call or letter asking for financial information?

You should never provide any financial information by phone unless you have verified the identity of the caller. Please report any suspicious calls or letters to us at 1-877-806-8964 (TTY 1-844-488-9722). You should also report suspicious calls received on your landline to the Federal Trade Commission (FTC) at 1-877-382-4357 and suspicious calls received on your cell phone to the Federal Communications Commission (FCC) at 1-888-225-5322. There is never any cost to you for participating in the Centennial Rewards Program.

What if I gave out my financial information?

Contact your bank or credit card issuer immediately if you gave out any financial information and are concerned that a phone call or letter was a scam. The Centennial Rewards Program will never ask you to provide any financial information.

How can I check my credit report?

You should check your credit report at least once a year and more often if there is any suspicious activity on any of your bank accounts or credit or debit cards. You are entitled to one free credit report per year. To request a combined report from the three major credit reporting bureaus, go to www.annualcreditreport.com.

What steps can I take to protect my identity?

Go to www.annualcreditreport.com to learn how to protect your identity, spot identity theft, and request credit reports.
