Reward Details

All ages, as recommended.
Must complete initial COVID-19 vaccination (1-dose vaccine, or both doses of a 2-dose vaccine), or a booster dose to receive this reward.
Reward available once every calendar year.
To earn reward points, you must either report on this site that you completed your vaccine or call us at 1-877-806-8964 (TTY: 1-844-488-9722) 8 a.m. to 6 p.m. MT, Monday-Friday. You will not receive reward points for completing this activity unless you report it.
Reward points can only be used to shop for items in the rewards catalog.
Reward points cannot be used to purchase alcohol or tobacco. Reward points have no cash value and cannot be converted into cash.
Activity must be completed between January 1 and December 31, 2023 to earn this reward.
