Centennial Rewards Terms & Conditions
Effective Date: March 31, 2022
The following terms and conditions apply to the Centennial Rewards program, including all Centennial Rewards websites, Centennial Rewards Member Services, and other Centennial Reward services.
By participating in the Centennial Rewards program and using the Centennial Rewards website, you accept these terms and conditions. IF YOU DO NOT AGREE WITH THESE TERMS AND CONDITIONS, YOU SHOULD NOT TAKE PART IN THIS PROGRAM.
- You are automatically enrolled in Centennial Rewards
The Centennial Rewards program is made available to you by your Medicaid health plan and the New Mexico Human Services Department (“HSD”). You are automatically enrolled in Centennial Rewards if you are a Centennial Care member. The Centennial Rewards program is administered by Finity Communications, Incorporated (“Finity”).
- Earn points for completing healthy activities
The Centennial Rewards program lets you earn reward points when you complete certain healthy activities. Go to centennialrewards.com to register or to find more information about the program. HSD may change the available rewards at any time. It is your responsibility to check the Centennial Rewards website or call us at 1-877-806-8964 (TTY: 1-844-488-9722) if you have any program questions or to view the latest terms and conditions.
- The terms and conditions may change
These terms and conditions may change at any time without prior notice to you. Any changes will take effect as soon as they are posted on the Centennial Rewards website. You should periodically check the Centennial Rewards website for updates. You agree to any changes in the terms and conditions by taking part in the Centennial Rewards program and then making use of the Centennial Rewards website after changes have been posted.
- Exclusions & restrictions may apply
The Centennial Rewards program may be subject to certain exclusions and restrictions. Your health plan and HSD make the final decision about rewards eligibility, reward criteria, and program rules. Your health plan and Finity reserve the right to suspend and/or cancel your participation in all or a part of the Centennial Rewards program at any time for any reason.
- Rules and restrictions
- You must be enrolled in Centennial Care
You must be enrolled in Centennial Care to earn points in the Centennial Rewards program. Your Centennial Reward points remain with you if you switch to another Centennial Care Medicaid health plan. If you are no longer enrolled in Centennial Care, you cannot earn new reward points. Any reward points you have already earned must be used within 12 months after your Centennial Care eligibility ends.
- Register to redeem your points
After registration, you can use your reward points to order healthy items from the Centennial Rewards Catalog. Go to centennialrewards.com or call 1-877-806-8964 (TTY: 1-844-488-9722) to register.
- Points expire at the end of the next calendar year
Centennial Reward points expire at the end of the calendar year following the year the points are earned. For example, any reward points earned in 2022 will expire on December 31, 2023.
- Points and rewards may change at any time
The incentives, points, program rules, and rewards offered through the Centennial Rewards program are SUBJECT TO CHANGE, TERMINATION, OR CANCELATION AT ANY TIME WITHOUT PRIOR NOTICE.
- Reward items may be replaced
If an item in the catalog is unavailable for any reason, the Centennial Rewards program reserves the right to replace it with another item of similar value. Alternatively, we may ask you to choose another item.
- No returns for items damaged during use
The Centennial Rewards program and Finity are not responsible for any damage caused to reward items after you have received them. Items damaged after receipt may not be returned.
- We are not responsible for harm
You must contact the product manufacturer directly if a reward item is defective. Your health plan, HSD, Finity, the providers of the Centennial Rewards program, and their respective employees, officers, agents, subsidiaries, and affiliates are not responsible for any harm that results from your use of a reward item, regardless of whether it is fully functioning or is defective.
- Damaged items must be returned within 10 days
Damaged items received must be returned within 10 days after the date of the postmark. Call us at 1-877-806-8964 (TTY: 1-844-488-9722) to request a return label or ask the carrier to return the package to sender. The item must be returned in the original packaging. We will replace the damaged item with the same or a similar item, subject to availability. Items damaged after receipt may not be returned.
- All reward orders are final
All orders are final. No returns or exchanges are allowed if you decide you don't like the item you ordered.
- Make sure to use your correct address
Items will not be re-sent if our tracking report shows that the package was delivered to the address you provided.
- Contact us if an order is not delivered
Call us at 1-877-806-8964 (TTY: 1-844-488-9722) if you provided the correct shipping address and did not receive your order.
- Your rewards may be taxable
The rewards you earn through Centennial Rewards may be subject to federal, state, and/or local income taxes. You are responsible for reporting any taxable amounts on your tax returns. You should speak with a tax advisor for more information.
- Centennial Reward points have no cash value
Centennial Reward points have NO CASH VALUE. The points are awarded to you for completing certain healthy activities. You can use reward points to purchase healthy items from the Centennial Rewards Catalog.
- Points may be canceled for fraud or dishonesty
The information you provide when you take part in the Centennial Rewards program and redeem points must be factual, accurate, and complete. By submitting information to the Centennial Rewards program, you agree that (1) you are at least the age of majority (or an emancipated minor) in your jurisdiction, (2) the information you are submitting pertains to yourself or someone for whom you are legally authorized to act (e.g., a minor child or ward), and (3) use of any information you supply will not violate any term of these terms and conditions.
Incentives, points, and rewards may be canceled if you break or attempt to break any rewards program rules.
- Reward inquiries
The Centennial Rewards program provides a system for reward inquiries. If you do not receive a reward that you believe you earned, call us at 1-877-806-8964 (TTY: 1-844-488-9722) to begin the reward inquiry process.
- Centennial Rewards does not provide advice
The Centennial Rewards program does not provide health, wellness, financial, legal, tax, fiduciary, or other advice.
- You must be enrolled in Centennial Care
- Taking part is AT YOUR OWN RISK
Your participation in the Centennial Rewards program is done AT YOUR OWN RISK. The Centennial Rewards program provides general information only. It is not guaranteed to be accurate, timely, or appropriate. Any health information, alerts, or wellness challenges are meant for general health information purposes only and are not intended to serve as personal health advice. You should consult your primary care provider or other qualified health care professional before participating in any exercise, nutrition, wellness, or prevention programs or activities, including Centennial Rewards programs and activities, whether or not you have a known health condition or risk factors.
- No medical advice, diagnosis, or treatment
The Centennial Rewards program does not provide medical advice, diagnosis, or treatment. You should not use any information provided by the Centennial Rewards program as a substitute for medical advice, diagnosis, or treatment. If you have a medical need, consult your doctor or other qualified health care provider. If you have a medical emergency, call 911 or go to an emergency or urgent care facility.
Your health plan, HSD, Finity, and the providers of the Centennial Rewards program, including their respective employees, officers, subsidiaries, affiliates, and agents, are not responsible for any injury, illness, death, or other outcome that may result if you take part in the program or any activities in the program.
- Compliance with laws & regulations
The Centennial Rewards program is designed to comply with applicable laws and regulations, including HIPAA. Finity does not accept any responsibility for the legal or regulatory policies and practices followed by HSD or your Medicaid health plan.
The Centennial Rewards program complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex.
- Centennial Rewards program intellectual property
The Centennial Rewards program name is the sole and exclusive intellectual property of HSD. The Step-Up Challenge, all technology provided through the Centennial Rewards program, and certain program design elements are the sole and exclusive intellectual property of Finity. The intellectual property rights for the Centennial Rewards program are protected by United States and international copyright and other intellectual property laws. All rights, title, and interests in the Centennial Rewards program are hereby reserved by their rightful owner.
Visitors to the Centennial Rewards website do not have any license or right to use any Centennial Rewards logos, copyrights, trademarks, or other intellectual property rights without the prior written permission of its rightful owner.
- Third-party content
Some of the content, tools, and calculators available on the Centennial Rewards website may be provided and owned by third parties. Any such third-party content is the exclusive intellectual property of its rightful third party. The Centennial Rewards program does not endorse, control, or make any representations about third-party content.
- Warranties
THE CENTENNIAL REWARDS PROGRAM AND WEBSITE ARE PROVIDED TO YOU "AS IS" AND "AS AVAILABLE," WITHOUT WARRANTY OF ANY KIND, EITHER EXPRESSED OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT.
NO GUARANTEE OR WARRANTY IS MADE FOR THE CENTENNIAL REWARDS PROGRAM OR THE CENTENNIAL REWARDS WEBSITE. Your health plan, HSD, Finity, and the providers of the Centennial Rewards program are not liable for any damage or injury to persons or property that may result from your use of the Centennial Rewards website or the Centennial Rewards program. We make no promise or warranty that you will be able to access or use any portion of the website. We make no warranty or guarantee that the site will be error-free or that the servers that make the site available are free of viruses or other harmful components. We disclaim any liability for any delay, failure, interruption, or corruption of any data or other information transmitted related to your use of the site.
Some jurisdictions may not allow the exclusion of implied warranties. This may mean that some of the above exclusions may not apply to you.
- Limitation of liability
By taking part in the Centennial Rewards program, you agree to release HSD, your health plan, Finity, the Centennial Rewards program administrators, and their respective affiliated companies and subsidiaries, officers, directors, employees, contractors, and agents, from any loss, claims, or damages that arise from your use of or participation in any portion of the program, whether known or unknown, suspected or unsuspected, disclosed or undisclosed.
In no event shall your health plan, HSD, Finity, and the providers of the Centennial Rewards program be liable for any costs, damages, or expenses (including, without limitation, any direct, indirect, incidental, punitive, special, or exemplary damages, lost profits resulting from lost data or business interruption, or health problems), that may result from your use or inability to use the website, or any products or services provided through the Centennial Rewards program, whether based on warranty, contract, tort, or any other legal theory, and whether or not we have been advised of the possibility of such damages.
Some states do not allow the exclusion or limitation of liability for consequential or incidental damages. The exclusion or limitation may not apply if you live in one of those states. The remedies under these program rules are exclusive and are limited to those expressly provided for herein.
- Consequential damages waiver
IN NO EVENT SHALL YOUR HEALTH PLAN, HSD, FINITY, OR THE PROVIDERS OF THE CENTENNIAL REWARDS PROGRAM BE LIABLE FOR ANY INCIDENTAL, INDIRECT, EXEMPLARY, SPECIAL, PUNITIVE OR CONSEQUENTIAL DAMAGES, LOST PROFITS OR LOST DATA OR INFORMATION DAMAGES, OR ANY OTHER INDIRECT DAMAGES, WHETHER ARISING IN CONTRACT, TORT (INCLUDING NEGLIGENCE), OR OTHERWISE RELATED TO YOUR USE OF THE WEBSITE OR USE OF THE REWARD ITEMS PROVIDED OR YOUR USE OF OR PARTICIPATION IN THE PROGRAM. This waiver includes, without limitation, any damages in connection with economic loss, physical or mental illness, recurrence of pre-existing physical or mental illness, injury, or death as a result of your use of any reward products or participation in any of the program activities.
- Hold harmless
You agree to hold HSD, Finity, your health plan, and the providers of the Centennial Rewards program and their respective officers, directors, agents, employees, and contractors harmless from and against any claims, actions, demands, liabilities, and settlements, including, without limitation, reasonable legal and accounting fees, resulting from, or alleged to have resulted from, your participation in the program, or use of any and all products, rewards, activities, or services offered through the Centennial Rewards program.
- Use the program only as allowed
You are not allowed to copy, reproduce, or redistribute the Centennial Rewards website or the Centennial Rewards program. You are not allowed to remove or change the website in any way, including but not limited to copyrights, trademarks, service marks, trade names, logos, or intellectual property.
- Governing jurisdiction and conflict of laws
The Centennial Rewards program is governed by the internal substantive laws of the State of New Mexico, without respect to its conflict of laws principles. By taking part in the Centennial Rewards program, you agree that any dispute, lawsuit, or controversy related to your use the program of will be resolved in the federal courts of New Mexico or the state courts of Santa Fe County, New Mexico.
You further expressly consent to the exercise of personal jurisdiction in the federal court of New Mexico or the state courts of Santa Fe County, New Mexico in connection with any dispute involving the Centennial Rewards program, Finity, or their affiliates, subsidiaries, employees, contractors, officers, directors, administrators, telecommunication providers, and other third parties.
- Severability
In the event any part of these terms and conditions is found to be invalid by a court of competent jurisdiction, that part shall be deemed invalid or modified as required by such court, and the remainder of the terms herein shall remain in full force and effect.
- Submission of unsolicited ideas or materials
The Centennial Rewards program is not responsible for any unsolicited ideas, proposals, or materials submitted to us. Any such unsolicited ideas, proposals, or materials will be treated as non-confidential and non-proprietary, and will not be returned to you.

Centennial Rewards Communications Terms & Conditions and Privacy Policy
- Telephone Terms & Conditions
You agree to allow us to contact you with periodic telephone communications from the Centennial Rewards Program that will provide information about the rewards program, important health plan and preventive services information, appointment scheduling support, and/or general health information. Telephone communications may be non-automated, automated, or pre-recorded. Message and data rates may apply. We may continue to contact you using non-automated telephone communications through the contact information you provide.
- Texting Terms & Conditions
You agree to allow us to contact you with one-time, periodic, or recurring text messages from the Centennial Rewards Program that will provide information about the rewards program, important health plan and preventive services information, appointment scheduling support, and/or general health information. You may always text STOP to stop messaging for that text messaging program or text HELP for help. Message and data rates may apply. Text messaging may not be available via all carriers.
- E-mail Terms & Conditions
You agree to allow us to contact you with one-time, periodic, or recurring e-mail messages from the Centennial Rewards Program that will provide information about the rewards program, important health plan and preventive services information, appointment scheduling support, and/or general health information. You may always opt out of receiving email messages by clicking the Opt Out link at the bottom of the message.
- Opting Out
You can opt out of certain communication mediums for the Centennial Rewards Program, including telephone calls, text messages, or emails in the My Account section of centennialrewards.com or by calling us at 1-877-806-8964 (TTY: 1-844-488-9722) 8 a.m.-6 p.m. MT, Monday-Friday. Centennial Rewards Program telephone calls may be recorded for quality control purposes.
- Limitation of Liability
Under no circumstances will we be liable for any direct or indirect, incidental, consequential, special, exemplary, or punitive damages arising out of or in connection with use of telephone communications and/or text messaging, whether or not we have been advised of the possibility of such damages.
- Communications Privacy Policy
Please notify us immediately if your mobile number changes. We are not liable for any communication or transmission of information by text that happens because you did not report that your mobile number has changed. Password-protecting mobile device(s) and enabling encryption, if available, is recommended.
Your consent to the Centennial Rewards Terms & Conditions and Privacy Policy allows us to process account registration; send program alerts and notifications; process rewards orders; reply to phone calls, emails, text messages, or other communications received from you; recommend wellness and program activities; provide you with a more personalized user experience; protect the legal rights of your health plan, Finity, and the Centennial Rewards Program; comply with applicable subpoenas, laws, and regulations.
